How Does Opt-Out Enrollment Work in a Section 125 Plan?
With opt-out (automatic) enrollment, every eligible employee starts in the plan, gets written notice and can choose to leave. Here is how it works and why participation matters to the employer.
Opt-out enrollment, also called automatic enrollment, means every eligible employee starts in the plan. Each one receives written notice of the plan and the payroll deduction before it starts, and anyone can choose to opt out. Nobody has to fill in a form to be covered.
Why does it matter?
Voluntary benefits that ask employees to sign up often see low take-up, even when the benefit is good for them. In a Section 125 plan, the employer's payroll tax figure is per enrolled employee, so participation is most of the number. Starting at 100% means the program works for the whole eligible team from the first payroll, without a sign-up campaign. Quantum 125 employers typically see 86–92% participation.
Is it permitted?
The IRS has described automatic enrollment in a cafeteria plan where employees receive notice and can elect cash instead (Rev. Rul. 2002-27). The plan document sets when an election or an opt-out takes effect. Your CPA reviews the plan document before you sign.
What does HR do?
Very little beyond the normal calendar:
- send the employee notice that Quantum Health Benefits prepares;
- attend one kickoff call with the implementation team;
- add new hires on your usual schedule.
QHB's enrollment team explains the plan to employees, answers their questions and handles opt-outs. Claims and billing questions go to QHB's team and app, not to HR.
What do employees see?
A notice, an enrollment conversation and then the benefits: telemedicine, prescriptions, hospital and accident benefits and a few everyday extras. Not major medical insurance; benefits vary by plan and state; see plan documents.
